This job description describes the typical responsibilities for the purpose of outlining duties and to determine job classification. It does not limit the Program Director or Executive Director from assigning other duties as the nature of the agency’s business and needs may change.
General Statement of Duties:
The Home Base Therapist works under the supervision and direction of their assigned supervisor, the Program Director and/or the Executive Director for the purpose of providing the highest quality of care to children and families. The Home Base Therapist provides structured, goal-oriented, time-limited therapy in the natural environment of families who need assistance recovering from physical, sexual, emotional abuse, neglect, substance abuse, mental illness, personality/behavior disorder, developmental disability, dysfunctional family of origin, and current family dysfunction. Maintains your own flexible work schedule per week. Available to families 24 hours a day, 7 days a week in crisis situations.
II. Conduct interventions that are consistent with the agency service model (values, beliefs, structure and practices) and with DCS Service Standards, including but not limited to:
- Schedule and complete client intake, which includes 24-hour crisis intake, intervention, and consultation seven days a week and must be provided primarily in the family’s home or provided on a limited basis at a community site.
- Complete ongoing risk assessment and monitor family/parental progress.
- Deliver time-limited services focused on limited objectives derived directly from the
established DCS/Probation case plan or Informal Adjustment.
- Develop short and long-term family goals with measurable outcomes.
- Deliver services that are family focused and child centered. They may include intensive in-home skill building and must include after-care linkage.
- Provide weekly/monthly progress reports; requested supportive documentation such as case notes, social summaries, etc.; and requested testimony and/or court appearances including hearings and/or appeals; case conferences/staffing.
- Have face to face contact with client within 48 hours of receipt of the referral or inform DCS/Probation if client does not respond.
- Prepare a written treatment plan and send to DCS/Probation within 30 days of contact with client.
Have written monthly report prepared and submitted to supervisor by the 1st of the month. Completion of EON and NCFAS information within 2 weeks of Intake.
- Meet the current guidelines on face-to-face client hours-currently at a 5/3 ratio. This includes client face to face time of five hours and 3 hours for other duties. Further clarification is that 62.5 % of your time is face to face and 37.5 % (true exact percentages-not rounded) is used for emails, travel, phone calls, monthly reports, staffing etc.
- Participate in other program areas as it is requested by your supervisor in order to meet program needs.
- The caseload will include no more than 12 active families at any one time.
- Service will be provided within the context of DCS practice model or Probation plan with involvement in Child and Family Team (CFT) meetings if invited.
- DCS may choose to select a standardized tool for evaluating family functioning. Services will include administration of this tool at the initiation of services as well as periodically during service probation.
- Complete all clinical and agency paperwork in a timely and accurate manner.
- 95% of all families that are referred with have fact-to-face contact with the client within 48 hours of receipt of the referral or inform the current Family Case Manager/Probation Officer if the client does no respond to the requests to meet.
- 95% of families will have a written treatment plan prepared and sent to the current Family Case Manager/Probation Officer following receipt of the referral within 30 days of contact with the client.
- 95% of all families will have monthly written summary reports prepared and sent to the current Family Case Manager/Probation Officer by the 10th of the month following the services.
- 67% of families that have a child in substitute care prior to the initiation of service will be reunited by closure of the service provision period.
III. Internal and external communication relationships, including but not limited to:
- Maintain confidentiality.
- Services will be conducted with behavior and language that demonstrates respect for sociocultural values, personal goals, life style choices, and complex family interactions and be delivered in a neutral-valued culturally-competent manner.
- Initiate and participate in individual consultation/supervision with supervisor at a minimum of 1 hour for every 25 face to face hours of services provided.
- Regularly participate in staff/group consultation meetings.
- Be familiar with service resources in the community.
- Understand the services, procedures, and regulations of the other systems with which client families come into contact, including the legal, social service, health care and school systems.
- Maintain compliance with all services standards set forth by DCS/and all other referral sources.
- Establish and maintain positive working relationships with referring agencies.
- Represent the agency in the community meetings and on interagency teams as directed.
Essential Function: Other
· Maintain prompt and regular attendance.
· On call 24 hours a day, 7 days a week to families.
· Arrange for coverage by other Therapist or supervisors if unavailable for an extended period.
· Schedule and work a flexible workweek to meet the needs of families, including evenings, weekends, and occasional holidays.
· Schedule family meetings at times that are convenient for family members.
· Provide back-up to supervisor and Therapists as needed.
· Travel to client home and other community locations and transport clients throughout the community.
· Attend meetings, training and continuing education activities as needed and directed.
· Establish annual goals and review periodically with the Program Director.
· DCS Probation satisfaction will be rated a 4 and above of the Service Satisfaction Report.
· 90% of clients will rate the services satisfactory or above. Providers are to survey a minimum of 12 clients or 20% of their caseload randomly selected from each county served.
· Masters degree with clinical license in social work, psychology, marriage and family therapy, or related human service field, and 2 years related clinical experience.
Valid driver’s license and automobile insurance.

· Knowledge of culturally responsive practice.
· Knowledge of crisis intervention, communication, parenting skills, and cognitive and behavioral interventions.
· Current First Aid and CPR Certification.
· Effective verbal and written communication skills.
· Familiarity with Microsoft Word, Microsoft Outlook and Internet. Able to write reports and treatment plans.
· Must live in designated service area.
· Exhibit an ability to effectively predict, recognize and/or assess potentially dangerous situations and/or persons, with ability to take appropriate action and resolve conflicts that can be violent and physical in nature.
· Ability to follow written and oral instructions.
